Background; Hepaitis B vaccination is recommended for all individuals with renal faifureNevertheIess, the response rate for this vaccine in hemadialy i patients is low. This study was designed to det&rnine, response rate to hepatitis B vaccination in CRFend ESRD patients and those factors that influence iL Material and Methods: We evaluated an iHBs level after prlmary v cdnatian (40ug intramuscularlyal Q,1, and & months) in 32 CRF (Serum Creatinine2mgIdL) patients on predralysis stage end 93 ESRD patients. H '~Ab, HIV- b, and NEs g were checked in all patients before vaccination beginning, and HBsAg positive patients were excluded. ntiFtBs liters were determined ire the period of 1 to 6 months after completion of vaccination. Results: SeroconversFon (antiHBs =1OrnlulmL) was found in 100 patients (8O ), but an excellent response (titer100 mlUmL) way observed only in 74 (59.2 ). Response rats was 71,9 and 82.8 in predialysis CRF rid ESRD patients respectiv Cy, but this difference wasn't sign:[icant (2-test; P=0.183), Predial sis patients showed n excellent response more th n dialysis patients (x'-test; P<IJ,05). Age, sex, initial serum cr atinine, anc smoking didn't influence response rate. Response rate in patientspatienis with diabetic mellitus was lower than others (62.2 vs. 87.5 ) (f-test; P-0.OQ1), and multiple logistic regression analysiss showed asignificant risk for vaccination r onre oonse when patients were drabetics (odds ratio 4.38; E5 confidence inter [: t7O~11- 4; P=0.002). Conclusion Our result showed that 1) there is a good response rate to hepatitis B vaccination in CRF and ESRD patiant , 2) hepatitis B vaccine nonresponders are more likely to have di hetes mellitus and 3) response rate in predialysis CRF patients is the same a the dialysis patients but predialysis CRF patients as compared with dialysis patients were more inclined to show an excellent response..
