BPH is a common problem that affects the men. Surgical procedures such as open prostatectomy and TURP are invasive treatment of BPH. Open prostatectomy may be more stressful and a pelvic intervention. Some patients report bowel complaints following prostatectomy in our practice that full filled with diagnosis of functional bowel disorders. Lack of previous evidence on correlation between IBS and BPH made us that do this study.The main objective of this study is comparison the relative frequency of IBS symptoms in the patients underwent prostatic surgery with control group. Methods and materialsIt was a prospective analytic case- control study that was done in Isfahan inyear 2004-2005. 66 patients who underwent prostatic surgery 12 weeks to 12 monthos ago as case group and 66 patients as control group were evaluated in this study.ResultsSymptoms suggestive of IBS were found in 7.5 in cases. The difference of relative frequency of IBS between control group and case group has no difference statically. (CI 95 , Pvalue = 0.117)In case group 21.2 of patients in open prostatectomy and 9 in TURP subgroup statically was seen. (CI 95 , Pvalue = 0.98, Odds ratio = 0.95411.306).Preoperatively diarrhea was seen in 25.8 of controls while post operatively diarrhea was seen in 38 of cases. The difference of diarrhea between case and control group was statically significant (Pvalue= 0.012). In cases, 51.5 of openprostatectomy subgroup and 24.5 of TURP had diarrhea. That difference of it between TURP and openprostatectomy statically was significant (pvalue = 0.026). Preoperatively constipation was seen in 2 1 of patients while the relative frequency of constipation in case group was 18 , that the difference of it has not statically significant (Pvalue= 0.16) the difference of it has not statically significant (Pvalue= 0.16).In the next part of study relative frequency of abdominal pain and improvement of it after defecation was evaluated, 15.3 of open prostatectomy patients and 9 of TURP patients had abdominal pain in the periode aftersurgery, that the difference between them was not statically significant ( Pvalue= 0.147).Another part of study was about association of bowel habit alternation such as diarrhea and constipation with onset of abdominal pain in control group. 15.3 of open prostatectomy and 9 of TURP cases had bowel habit alternation in the periode after surgery, that difference of it between them statically was significant ( Pvalue = 0.047).DiscussionOn the basis of this study, relative frequency of diarrhea in the patients underwent open prostatectomy was greater than control group and the patients underwent TURP.Bowel habit alternation after open prostatectomy increased.Indeed open prostatectomy increased the relative frequency of diarrhea and bowel habit alternation. But the relative frequency of IBS, constipation and abdominal pain in contral group and case group was not differ statically. Finally we resulted that openprostatectomy is associated with increase of diarrhea and bowel habit alternation.Key wordsIBS,TURP,Open prostatectomy , Diarrhea.
