Abstract : Introduction: The risk of infection with transfusion-transmitted viruses has been reduced remarkably. However, a zero-risk blood supply remains a popular goal. Some authorities have introduced the screening for antibody to HBc (anti-HBc) as a surrogate test for the presence of several infectious agents. The aim of this study was to evaluate the prevalence of HBV infection markers and the potential value of anti-HBc testing of donors to detect occult HBV infection.Materials and methods: In this descriptive cross-sectional study, between April and Jun 2004, 545 blood units were collected and tested by HBsAg ELISA kit, then all HBsAg negative samples were tested by Anti-I-Mc ELISA kit. To detect occult HBV infection all HBsAg negative and Anti-HBc positive samples were tasted by PCR.Results: All samples were HBsAg negative but 43 blood units were Anti-I-Mc positive (8 ). These blood units were tested for HBV DNA by PCR. Five blood units were HBV DNA positive.Discussion: Occult HBV infection is a clinical form of HBV infection that HBsAg is not express by HBV and blood units cannot be screened by usual method (ELISA), therefore more sensitive techniques are needed. PCR is a sensitive technique that detects HBV DNA even in a small mount. Our results clearly demonstrated that more sensitive tests and complementary tests such as PCR and AntiHBc, respectively, are essential to ensure safety of blood units..
