Abstract : Iron accumulation due to diserythropoiesis and regular blood transfusion in thalassemia major patients is a major problem in Mediterranean as well as in Iran. At normal levels iron is known to be an essential mineral for human. However excess iron (iron overload) has many disadvantages: it enhances oxidative stress in cells of immune system leading to biological molecules malfunctioning; it impairs host defence mechanism by directing influencing the T-lymphocyte function. Iron mainly in its nonت‍protein bound, low molecular weight form, cause cellular damage by participating in the generation of the hydroxyl radical, thought to be the principal effector of oxidative DNA damage. Telomeres, as triple-G-containing structures, are highly sensitive to damage by oxidative stress. Accordingly, high-intensity stresses can cause telomere shortening without DNA replication by inducing telomeric double-strand breaks at high frequency. One possibility is that telomerase is essential for the repair of telomeric DNA following damage by oxygen radicals. Iron chelators are presently used in attempts to prevent the longت‍term consequences of iron overload. Deferoxamine is the most widely used agent, but has significant disadvantages, including its high cost, parenteral method of administration, toxicity, and frequent noncompliance, leading to a search for a safe, orally active alternative. Methods: Peripheral blood mononuclear cell (PBMC) were isolated from 20 patients with thalassemia major and 20 healthy donors. To determine if Fe loading of cells occurred, 1 ml of lx105 cells/well were seeded in a 24-well plate in presence of 500 piM FeSo4-7H20 (0.139 mg/ml) and incubated for 24 h. Cells were stimulated with PHA and treated with Deferoxamine and Silymarin for 72 h. Telomerase activity was measured by the telomeric repeat amplification protocol-based telomerase polymerase chain reaction enzyme link immunosorbent assay at 0 and 72 h of incubation..
