Abstract : Introduction: Rheumatoid arthritis (RA) is the most common inflammatory arthritis, affecting about 1 percent of the general population worldwide and attacks all ethnic groups. The rheumatoid arthritis can occur at any age, but its prevalence increases with age; the peak incidence is between the fourth and sixth decades. The annual incidence rates per 100,000 members of the population were 35.9 female cases and 14.3 male cases. Whenever Rheumatoid Arthritis (RA) is suspected the presence of Rheumatoid Factor (RF) is investigated. The presence of RF is one of the diagnostic criteria for RA listed by the American Rheumatism Association. Classical RF, according to Waaler-Rose, consists of antibodies to rabbit immunoglobulin (Ig). Although RFs have generally been associated with persons who have rheumatoid arthritis (RA), they are also present in normal individuals. Titers are elevated as well in patients with a variety of other diseases, including viral infections, acute and chronic inflammatory diseases, and lymphoproliferative diseases (such as chronic lymphocytic leukemia, Waldenstrom's macroglobulinemia, and mixed cryoglobulinemia). On the other hand, It has been established that antibodies precisely detecting Mendelian Genetic markers of human Ig in the Gm system are common in RA and are rare in other diseases. Estimate of the occurrence of anti Gm allotypes in RA is that more than 95 of Caucasian RA patients and statistically significant differences were found in Gm system allotypes which more frequent in rheumatoid patients than in healthy subjects. Materials & Methods: Serums of 217 patients (RA) and 270 healthy individuals were evaluated with anti Gm and G 1 m(x) allotype. Evaluation of anti Ig allotype (anti-Gm) was done by haemagglutination assay with blood group 0 cDE/cDE or CDe/cDE (screened with panel of c, C, e, E, D cells Cat. No. PS 38546 from Blood Fractionation and Research Co.) and pooled anti-D serum that isolated from immunized individuals with Rh antigen by plasmapheresis and for Glm(x) typing by competitive ELISA with Avidin-Biotin system, Glm (x) antibody (Batch No. 2984 from CLB Co.) was conjugated with biotin and micro titer wells were coated by anti and Gim (x) antibody (Batch No. 4199 from CLB Co.)Results: The number of patients that were positive in anti Gm measurement are 210 of 217 individuals and 7 of 217 individuals were negative (96.8 positive and 3.2 negative) and for healthy individuals were 17 of 270 individuals positive and 253 of 270 individuals were negative (6.3 positive and 93.7 negative) and The number of patients that were positive Glm(x) typing are 52 of 217 individuals and 165 of 217 individuals were negative (24.0 positive and 76.0 negative) and for healthy individuals were 48 of 270 individuals positive and 222 of 270 individuals were negative (17.8 positive and 82.2 negative).Discussion: Our findings show that There are statistical significant differences between specificity and sensitivity of anti Gm antibody and RF test (anti Gm antibody measurement more specific adn sensitive) (p < 0.001)) and there is difference between patients and healthy individuals (Glm(x) allotype more frequent in rheumatoid arthritis patients than in healthy individuals ) but this difference was not statistically significant (p value was 0.093)..
