Abstract

Introduction: Identifying histological stages of disease and making decision about antiviral therapy in chronic HCV infection is based on liver biopsy. We conducted this study to assess whether a discriminant model consisting routine laboratory and radiologic data could be used to differentiate patients with active hepatitis, who are candidates of antiviral therapy, from those with inactive hepatitis and cirrhosisMethods. A total of 162 patients with documented chronic hepatitis C infection were enrolled. Serum AST, ALT, albumin, total bilirubin and platelet count as well as the results of clinical examination and sonography findings were collected. Two distinct logistic. regression models were used to identify patients with inactive hepatitis andResults. The specificity of logistic regression model for detecting inactive hepatitis was 96.4 , while the sensitivity was 71.4 . The positive and negative predictive values were 71.4 and 96.4 , respectively. The overall accuracy was 93.5 . The specificity of the other model for detecting cirrhosis was 100 and sensitivity was 88.9 . The positiveand negative predictive values were 100 and 98.2 , respectively. The overall accuracywas 98.4 .1 Conclusion. In a considerable proportion of patients, noninvasive monitoring of chronic hepatitis C could be made using routine laboratory tests and ultrasound after exclusion of patients with strong clinical evidences of decompensated cirrhosis. This hypothesis must be validated through a multicenter study with a larger series of patients..
