Hepatitis B is an important health problem and a major cause of acute and chronic hepatitis, cirrhosis and hepatocellutar carcinoma. Approximately 30 of the world population (1.8 billion people) have the serologic evidence of HBV infection of whom 350 million are estimated to suffer from chronic HBV (hepatitis B virus) infection; at least 504,00O chronically infected people die of liver malignancy and cirrhosis each year. According to Iranian studies, about 22 to 37 of general population in Iran are HBcAb positive and about 1, to 8.69 of the population are chronic HBV carriers.Sexual contact is one of the common routes of HBV transmission. Almost all pre marriage individuals (those considering legal marriage) are obliged by Iranian law to undergo a predefined battery of screening tests in government-designated laboratories; this can make pre marriage individuals an accessible group for a preventive intervention. Finally, since most pre marriage individuals are in young age groups, they have a rather long life expectancy, which allows enough time for them to suffer from chronic complications of HBV infection in productive years of life. We decided to perform this study to provide health policy makers in Iran and those countries with similar demographic conditions (especially in the Middle East) with a utility analysis of pre marriage screening of hepatitis B. Objectives: In this survey we were to determine the youth attitude toward pre marriage testing for Hepatitis B and also influencing and associating factors, METHOD: Through a cross-sectional study with a convenience sampling we took 1316 participants from pre marriage youth which came to premarital courses of marriage health (the courses are mandatory in Iran). Data collection was by a questionnaire and answering to it was voluntarily, the participants answered the questions after reading the other side of the questionnaire which contains information about HBV and liver disease, modes of transmission, the infection attack rates, preventive methods and their efficacy. The questionnaire was performed in form of self-administration and the questions were close-ended offering a choice among "yes", .'no", or 1 don't know" answers. RESULTS: The sample population was 1316 people coming to pre marriage courses half man, half woman, With the age average 24.39 4.63Mean ▒ standard deviationn.The participants had good attitudes towards premarital testing for HBV (73.2 ) but their practice was disappointingly low (18.6 ).We found out a significant association between Attitude and Education level of participants. Another difference was between men and wornen Attitudes, Most of factors that were collected in the questionnaire had significant as oci tion with the main attitude that wiLl discus later.DISCUSSION:The factors we found out to influence the attitude of participants towards pre marriage screening of Hepatitis B are as below:Concerning about the test result, concerning about their finances find them HBV positive, concerning about their family reaction to their positive test, Concerning about their marriage course, their difficultj to make decision for marriage if their finances testwould be positive, and their difficulty to make decision for marriage if their test would be positive, having knowledge about disease.The big difference between attitude and practice shows a big social and cultural problem.The difference between men and women attitude and the role of education arid also Fear of stigmatization and social reaction should be mentioned. Cur study as mentioned above indicates that a research based 1E (1nforrnation, Education, Communication) activity need to be implemented in an organized and continuous manner before pre marriage screening of Hepatitis B (or maybe for other diseases) getting nationwide. Implementing strategies to increase knowledge of HBV, in addition to reduce stigmatization may increase the acceptance of HIV testing.Further research, especially qualitative studies,would be valuable to obtain in-depth information about the reasons whysonic people do not accept premarital F-[1V counseling and testing..
