Introduction: In critically ill patients who suffer acute respiratory failure, predicting the need for mechanical ventilatory support and weaning are of great importance. Non-respiratory factors such as metabolic and nutritional ones have not been fully characterized. Albumin and glucose are two parameters of nutritional and metabolic states and the role of hypoalbuminemia and hyperglycemia in morbidities and mortality in ICU population have been demonstrated in previous studies. Patients and Methods: This was a retrograde, analytic-descriptive study on 600, over 16 patients who required ICU care for more than 72 hrs. Patients with serum albumin level of <3.3 g/dl were considered hypoalbuminemic and patients with serum glucose level of >200 mg/dl were considered hyperglycemic. Using Chi-square and Fisher's exact test we compared the need for mechanical ventilatory support and it's duration between normal patients and hypoalbuminemic or hyperglycemic patients. Results: The mean serum albumin and glucose concentration were 3.18 g/dl and 168.35 mg/di respectively. 72 of our patients went under mechanical ventilatory support and the mean duration of ventilator dependency was 6.08 days. The comparison of ventilator attachment between patients with normal albumin and hypoalbuminemic patients was significant.( P value= 0.005); but it was not significant between patients with normal glucose and hyperglycemic patients and between normal albumin normal glucose patients and hypoalbuminemic hyperglycemic patients. Comparison of ventilator dependency between those groups mentioned above were not significant. Discussion: Initial serum albumin and serum glucose concentration did not necessarily predict the need for mechanical ventilatory support and weaning success. Further prospective clinical trials which determine profile of changes in I've thought so much; I've searched a lot. The meaning of life was the ultimate goal of this aimful search. I walked on both sunny and rainy days. It was sometimes full of burden; sometimes full of ease. Till now 1 haven't found anything more valuable than love. It actually imposed itself on me. The more I searched, the more manifestations of love appeared to me, but I knew deeply inside that even these beautiful views are not the case. The search will be continued. The best meaning will be catched near the grave. This dissertation is dedicated to all who are trying to find the real meaning of life in an endless search.serum albumin and glucose is needed to verify the predictive value of daily measured albumin and glucose on weaning process..
