Introduction: Refractive errors have a wide distribution and nowadays, greater numbers of patients are hoping to dispose of their spectacles or contact lenses using refractive surgeries. One of the most prevalent complications of these surgeries are night vision disturbances such as glare and haloes. preoperative pupil size have an effect on the patient's perception of glare and halos. In fact, large pupils contribute to an increase risk of post-refractive night vision disturbances and patients with larger pupil diameters report these disturbances more frequently. Refractive surgery, by changing the corneal refractive power, could modify the measurements of the patient's pupil and cause change in the refractive zone-pupil size ratio and therefore can become a problem for those patients who complain of pupil-related night vision disturbances after surgery. This study is designed to compare the mean pupil size before and one month after P.R.K. The results of this study can help us in better determination of the ablation zone regarding to pupil size before P.R.K and therefore decreasing the complications of P.R.K. Methods: This was an interventional prospective study which was done in spring of 1385 on patients with myopia who underwent P.R.K for correction of their refractive error. The sample size was 110. The pupil size of patients were measured before and one month after surgery. Atlast, the mean pupil size before and one month after P.R.K were measured and they were compared with pair t-test tes. Results:Unlike the results of previous studies, we have a relative myosis after P.R.K(pvalue=0.001). there was a significant change in pupil diameter in female patients but there was no significant difference between pupil size before and after P.R.K in male patients. In refractive errors less than 6 diopter, we have a relative myosis but there is no significant difference between pupil size before and one month after surgery in refractive errors more than 6 diopter. Conclusion: In patients who underwent P.R.K, we have a relative myosis which is the same as the theory. In the present study, correction of refractive errors less than 6 diopter, cause a significant change in pupil size (P-value= 0.001), but in correction of refractive errors more than 6 diopter, there was no significant change in pupil size.
