Abstract

Introduction: Diffuse large B cell lymphoma (DLBCL) is the most common type oflymphoid malignancy and accounts for 30-40 adult non-Hodgkin lymphoma (NHLs). Approximately one third of the non-Hodgkin's lymphomas arise in tissues different from the lymph node, and for this reason, they are usually termed extra nodal lymphomas. It has been shown that the incidence of lymphomas has increased, and that extra nodal lymphomas increased more rapidly than nodal. We performed this study to get aware of the stage of our patients diagnosed With DLBCL in Isfahan province and our results help us compare the prognosis and survival of nodal DLBCL and extranodal DLBCL patients.Materials and methods: This is an observational descriptive-analytic study. All the patients diagnosed with DLBCL admitted in Seioddoshahah hospital between1375-1382 enter the study. The patients who were diagnosed before or had relapsed were excluded from the study. We collected the data from each patient in a specific from. The time of diagnosis and also the time of expiration was recorded for each patient and calculated for the survival. Finally to analyse the difference between these two groups we used spss ver.13 software and independent T test and Mann-whitney and Chi-square tests. Results: In total 100 patients diagnosed with DLBCL were collected. The mean age of patients was 48 years (std 18.02). 45 patients were female and 55 patients were male. In total 25 patients had stage 1, 23 patients had stage2, 25 patients had stage3 and finally 24 patients had stage4. 70 cases (72.2 ) had nodal disease and 27 cases (27.8 ) had primary extranodal disease.finally after three years almost 60 percent of the patients were alive in extranodal lymphoma but only 20 percent of patients were alive in nodal lymphoma group. Conclusion: Our results indicate that the prognosis of patients with nodal DLBCL is poorer than patints with extranodal DLBCL patients and the difference is statistically significant. (p=0.002). Key Words: Diffuse large B-cell lymphoma, nodal, extra nodal, survival.
