Abstract : ntroduction:Multiple sclerosis (MS) is a chronic inflammatory demyelinating and disabling disease of the central nervous system of presumed heterogeneous but still unxnown causes, with the onset of symptoms usually occurring between the ages of 20 and 40. Prior studies have shown that early onset MS (onset < 16 years; EOMS)) occurs in 3 to 5 of all MS cases. In the past, MS was often not considered in the differential diagnosis when a young patient presented with neurologic sign and symptoms. As potentially disease modifying therapies evolve in the management of this disease, it becomes even more important to better understand the natural history of MS in such cases. Method & Materials:It was a descriptive and cross-sectional study which was done in Isfahan MS clinics since Jan of 2006 until Jun of 2006. In the analysis of MS patients from our MS clinics, we screened 227 patients, which was 104 of them showed on age at onset less than 16 years, and 123 of them in adult age's. Demographic factors of both EOMS and AOMS patient's were gathered by a guestionnaire and the clinical disability of patieut's were determined according to EDSS criteria which was done by neurologist.Results: In the EOMS group 3.8 of patients had onset at age 10 or younger, 18.2 were at age 10 to 14, and 77.8 were at the age of 14 to 16. the female/male ratio was 4.47:1 in EOMS while it is 3.92:1 in AOMS. Parasthesia is the most frequent initial symptom in AOMS while it is optic neuritis in EOMS. Relapsing - Remitting is the frequent type of progression in both MS group's. Periventricalar plaque is the most frequent finding of brain MR1 in both MS groups. 80.8 of EOMS patients and 84.6 of AOMS patients has EDSS of less than 3.5. Conclusion: The data obtained in this study do not support the hypothesis that course and progression of disease in EOMS is much slower than AOMS. But we can find a significant difference between duration of disease and degree of disability in AOMS which is like a lineor relation, but this is not true for EOMS.
