Introduction: Benign prostate Hyperplasia (BPH) is one of the most common causes of obstructive voiding symptoms in elderly men. Benign prostate Hyperplasia (BPH) is composed of epithelial and stromal elements; the parentage of these elements varies in different individuals. Prostate specific Antigen (PSA) is one of the prostate epithelial secretions in semen, which plays a role as a semen liquefier. PSA is found in small amounts in serum. According to different prostate volumes in different individuals (which means variable prostate epithelial volume), PSA levels varies in different individuals, So to compare PSA levels in patients , the ratio of PSA to gland volume is used which is termed the PSA density.As PSA is secreted by prostate epithelium, a relationship between PSA density and epithelial element percentage is probable; this probability is evaluated in our study. If such a relationship is found, PSA density will be a reliable parameter to predict prostate epithelium Volume in future studies.Objective: To determine the relationship between PSA density and prostate epithelial element percentage in BPH.Patients and methods: The present study is a prospective one. Patients with BPH who were candidates for open prostatectomy in Alzahra hospital could be included in our sample size:Sampling was done by simple nonrandomized method; as a pilot research. fifthy cases were considered for the sample size. Serum PSA level determination, Transrectal Ultrasonography for assessment of prostate volume and prostate epithelial element percentage evaluation with our pathologist colleague were the steps of a process for every case before and after open prostatectomy of correlation, between PSA and epithelial element percentage and also between PSA density and epithelial percentage was calculated at the end.Results: Out of 50 cases with average age of 59 ▒ 6.6, PSA density average was 0.09 ▒ 0.03 with maximum of 0.15 and minimum of 0.03; the average of prostate epithelial element percentage was 56.62 ▒ 15.7 with maximum of 80 and minimum of 30 .Coefficient of correlation was 0.91 for variables of prostate epithelial element percentage and PSA density and 0.75 for PSA and prostate epithelial element percentage variables.The following linear functions were found for the above-mentioned pairs of parameters.PSA = 0.078 (epithelial element percentage) ت 0.762PSA density = 0.002 (epithelial element percentage) ت 0.018Conclusion and discussion: The results signify the predictive value of PSA density for prostate epithelial element percentage. This concept leads to consider PSA density as a predictive parameter for epithelial element percentage in the studies in the field of natural history of BPH and efficacy of different medical therapies of BPH.
