Abstract :Introduction: Many intracranial structures are involved in the pathophysiologic process of migraine with trigeminal nerve (TN) being the main one . in purpose of evaluating and determining the effects of greater occipital nerve (GON) blockedge on migraine behavior and comparing this effect with oral prophylactic antimigraine drugs specially Nortriptyline , we establish this study . Methods : Using a double blind cross over study , we submit these two groups of patients , one a GON blockade with Bupivacaine 0.5 (BP) and the other Nortriptyline (NO) . the clinical effects of these two therapeutic methods have been evaluated subjectively through a pain analytical visual scale (frequency , duration and intensity of migraine attacks ) during three months after treatment and the results compared statistically . Results: 30 patients given BP and 31 patients given NO demonstrating significant beneficial effects on the frequency , duration and intensity of the attacks in BP group in comparison to NO group (p < 0.000)).Conclusion:The conclusion is that the anesthetic blockage with BP on the GON decreases the number of crises, their duration and intensity more and sooner than NO, therefore we can use this procedure as an alternative choice for migraine, specially in patients that can not tolerate oral drug side effects..
