Abstract : Introduction:Ovarian masses are commonest pelvic mass in women. The reason is the high neoplastic potential of this organ. Two thirds of the ovarian masses occur among women with 20-44 years of age. The risk of developing ovarian cancer lifelong among women with no special risk factor is 1.5percent. Most ovarian masses are benign ( 80-85percent). But the rest 10-15 percent to be malignant are the most mortal gynecologic cancers worldwide. So that ovarian cancer is the fifth cause of death due to cancer in American women and its overall mortality is the highest of all other gynecologic cancers. According to this high mortality rate. the necessity of this study is well seemed. Methods & materials:This descriptive-analytic cross-sectional study was performed over women who had undergone surgery for their ovarian mass by a gynecologist in Beheshti and Alzahra hospitals of Isfahan during 1999-2002. After searching in medical records of the hospitals, 374 files were found to have the inclusion criteria of the study. The extracted data would be entered in the data collection forms by a general practitioner for each of the patients and then analyzed by SPSS software.Results:Commonest cancer for age range below 20 years of age was germ cell tumors (80percent).76percent of ovarian benign masses had occurred in age range 20-44 years of age.82percent of pri mary cancers at age range of 20-44 years of age were of the epithelial type. Amongpatients'chiefcomplaints,chronic abdominal pain (44percent), sensation of distention (36.7percent.), dyspepsia (8percent) and the presence of cancer was a statistically significant relationship.In sonographic evaluation of pure ovarian cysts, a relationexistedbetweenseptation(48percent),multilocularity (67percentl, wall irregularity (30percent) findings and the presence of malignant mass.70percent of ovarian cancers which had undergone surgery didn't have any staging.Discussion:According to the literature, around 20percent of ovarian tumors are malignant while in this study the number of benign tumors was only two thirds of the malignant group. Ovarian cancer risk increases in age range of 40-45 years of age and the peak is at 55-65 years of age. In this study, 74percent of cancerous patients were over 45 years old and the mean age of cancerous patients was 60 years of age which correlates to the current literature. In this study, 77percent of patients with benign masses were at age range of 20-44 years of age which well correlates to the literature that nearly two thirds of ovarian benign tumors occur among those with 2044 years of age. According to this study findings, epithelial cancers were the commonest for those over 20 years of age and according to the fact that there was no staging for most cases and therefore no surgery performed as the cornerstone therapeutic procedure in encountering patients with this mortal disease, basic corrections in management of such patients seems logical and very necessary..
