Abstract : Background/Aim: Preemptive analgesia is a technique, applied before noxious stimulation, result in lower pain intensity. Receptors of the Nmethyl-d-aspartat (NMDA) is involved in this mechanism. Ketamine is a NMDA receptor blocker, which has been used to reduce centeral sensitization. This study is aimed to evaluate the preemptive effect of low dose IV ketamine (0.5 mg/kg) on postoperative pain after appendectomy. Methods and Patients: We investigated 80 patients randomly assigned to get low-dose of ketamine in i.v. administration (40 patients) or placebo (i.v. saline solution) (40 patients) before appendectomy. patients of ketamine group received 0.5 mg/kg of ketamine IV 10 min before surgical incision and patients of control group received saline 10 min before surgical incision. surgical and anesthesia protocol were same in both groups. In the postoperative period, if needed, analgesia was maintained with IV pethedine. Pain was assessed by VAS (Visual analogue scale) at TO (arousal), and T4, T12, 724 later. The.time interval for the first request of analgesia and the total amount of analgesic consumption were reported. All data were stored using SPSS and two independent samples T test was used. Results: There was a statistical difference between 2 groups in the average mean values of pain intensity at 10 and T4, T12, 724 in the numerical scale. VAS was statistically significant lower in ketamine group (p <0.05) vs. control. The time interval to request the first analgesic supplement was longer in the ketmine group than control (23.12 min versus 18.12 min; p <0.05). There was a statistical difference between two groups in the total dose of pethedine consumption (p <0.05). Conclusion: In this study, pre-incisional ketamine had a preemptive effect in reducing pain after appendecyomy . Key words: low dose ketamine - appendectomy - VAS ت preemptive analges.
