The study population is considered of the patients who receipted in hair transplantation clinic of yordan in Tehran from 1999 to 2006 and they were 345 patients. The past history of smoking ( duration, cigarette's count smoking per day), also the results of visits that have done 2 weeks, 1 month, 3 months and 1 year after the surgery have been gathered from patient's fifes. Then after entering the spss program it evaluated with Kendall's tau-6 test. Results: Among 345 patients who evaluated in this study 112 patients (32.5 ) were smokers, and 233 one (67.5 ) were non-smokers. 4.5 of smokers and 3 of non-smokers had infection, 2 weeks after the surgery. None of the patients of both groups had bleeding, 2 weeks after the surgery which this is 0 of them. 7.1 of smokers and 5.2 of non-smokers had scar, 2 weeks after the surgery. 2.7 of smokers and 2.6 non-smokers had hematoma, 2 weeks after the surgery. 8.9 of smokers and 5.2 of non-smokers had scar, 1 month after the surgery. 3.6 of smokers and 1.7 of non-smokers had hematoma, 1 month after the surgery. 11.6 of smokers and 3 of non-smokers had scar, 3 months after the surgery. 1.8 of smokers and 0 of non-smokers had hematoma, 3 months after the surgery. There was no signcant relation between cigarette smoking duration and 2 weeks after surgery: infection, scar formation, and hematoma; 1 month after the surgery: scar formation, and hematoma; 3 months after the surgery: scar formation, and hematoma. Also there wasn't any signcant relationship between cigarette's smoking per day and 2 weeks after surgery: infection, scar formation, and hematoma; 1 month after the surgery: scar formation, and hematoma; 3 months after the surgery: scar formation, hematoma. Hair loss reduction percentage in smokers group, 3 months after surgery was 20.94 , and in non-smokers group, was 23.28 . .Hair Loss reduction percentage in smokers group, 1 year after surgery was 93.87 , and in non-smokers group was 96.30 . Although there was no significant relationship between smoking duration and hair Loss reduction after 3 months, and after 1 year.Discussion and Conclusion: Later some studies have done to show the relationship between smoking and hair loss and also other organs transplantation's outcome. Although in this study we found a rising degree of complications after the surgery in smokers and a decreasing degree of hairloss reduction percentage in them, but we didn't find any signcant static relationship between smoking and hair transplantation surgery outcome and surgery complications..
