Abstract : Background: Diabetes mellitus is one of the most common and serious chronic diseases in the world. Despite previous studies have reported contradicting results regarding hearing impairment in diabetic patients and its relationship to other diabetic complications and to metabolic control, the aim of the present study was to examine the auditory function in a group of children with type 1 diabetes mellitus and to evaluate its relationship with the duration of the disease and the tightness of metabolic control. Methods and materials: The study is descriptive analytic study that was carried out as case-control study. One hundred children and adolescents between the ages of 5-18 years with type 1 diabetes mellitus attending the Esfahan Diabetes Research Center (Sedegheah Tahreah) were investigated together with one hundred age and sex matched non-diabetic controls. Then, the patients were referred to the audiometry center to assess their hearing thresholds by pure-tone audiometric tests. Both air and bone conductions were tested at frequencies between 2508000 Hz arid 250-4000 Hz respectively. Hearing impairment was noted at auditory threshold above 25 dB in any frequency. Results: The auditory threshold of the diabetic patients was uniformly higher than that of the control group in all frequencies, then the difference assumes statistical significance in all frequencies (from 250 to 8000 Hz) (P <0.05). 21 patients (21 ) had satisfied our definition of significant hearing loss with auditory threshold above 25 dB in at least one frequency. When the degree of hearing impairment was quantified, 8 of the 21 patients (38.1 ) had mild hearing loss (range 25-40), 11 patients (52.38 ) had moderate hearing loss (range 41-65), and 2 patients had severe hearing loss (>65). The hearing loss was symmetrical, and affects both sexes equally. Duration of diabetes and HbAic concentration showed positive correlation with the increased hearing thresholds; while the auditory thresholds were not associated with the sex of the patients. Conclusion: In conclusion, children with type 1 diabetes had hearing loss, which was cat present in their matched controls. Such changes were not related to the gender but it was associated with the degree of metabolic control and duration of the disease. This complication can be prevented or delayed by strict glycemic control, so that it is suggested to perform audiometry for all diabetic patients who have poor metabolic control and duration of diseases more than 5 years.
