GCS is one of the most important scale for evaluating the depth of the coma, tracking the patient's progress, and predicting, somewhat, the ultimate outcome of the coma in head injured patients. The focal nature of poisoning and rapid changes of consciousness level brings into question the validity of the GCS as a measure of level of consciousness and a predictor of outcome in poisoning. Previous study showed that for the assessment of mortality, the GCS score provides the best indicator for some poisoned patients, because of simplicity, less time-consuming and effectiveness in an emergency situation. Until now there is no investigating about the prognostic value of the total GCS score and its individual components in a group of patients with mixed drugs poisoning (MDP). Since mixed drugs poisoning is the most common cause of poisoning in the poisoning center of Noor hospital in Isfahan, the prognostic value of GCS and its components in predicting outcome of patients with MDP were investigated. Main Objective:Determination of prognostic value of GCS and its components in predicting outcome of patients with MDP in poisoning center of Noor hospital, Isfahan, Iran. Methods: In a cross-sectional study following prospective data collection in poisoning center of Noor Hospital, patients with MDP admitted from July to October 2007 were evaluated. Medical staff prospectively recorded demographic data and clinical features of patients on admission, including measurement of the eye, motor and verbal scores and total GCS score. Data were recorded prospectively on admission to the poisoning center of Noor Hospital by medical staff in an appropriate form. The outcome was followed based on patient's files. Outcome recorded in three categories: survived without complications, survived with complications and death. Data were analyzed using SPSS 13 and MedCalc statistical software. P < 0.05 was considered significant. Independent sample t-test, stepwise logistic regression, and ROC curve were used for statistical analysis. Results: During the study, 152 patients with inclusion criteria were evaluated. Poisoning was common in women than men (66.4 versus 33.6 ). The average ages of women and men were 24.44 f 8.06 and 24.96 ▒ 6.72 respectively. Majority of the patients (85.5 ) survived without complications and stayed in the hospital for less than 24 hours (74.3 ). Regarding to outcomes, there was a significant difference between mean value of each component as well as total GCS in patients with MDP. The most important factors in prediction of outcome were GCS, verbal and motor components. There is a negative but not strong correlation between total GCS and each component individually with outcomes. There was a not significant difference between GCS and the verbal and motor component in area under the ROC curves. Discussion: Total GCS score and its verbal and motor components can be employed to determine the outcomes of patients with MDP.
