INTRODUCTION: Chronic tension-type headache (CTTH) is a prevalent disease with great disturbances in the quality of life. It is one of the primary headaches most difficult to treat. Despite existing controversies, tizanidine, an alpha 2-adrenergic agonist is an effective agent in the preventative treatment of chronic tension-type headache. This study was conducted to evaluate the efficacy of tizanidine in CTTH. METHODS: One-hundred patients with chronic tension-type headache were included in a randomly assigned, double-blind, placebo-controlled, crossover trial. None of the patients had migraine, depression, opium addiction or analgesic overuse. Tizanidine or placebo 8 to 12 mg/day was given for 12 weeks. Follow-up visits were performed at 2-week intervals. RESULTS: Eighty-nine patients completed the study. The calculated average of headache duration x headache intensity during treatment with tizanidine (867.84) was lower than during treatment with placebo (1124.92) (P<0.01). Headache frequency (P<0.01), headache duration (P<0.01), and headache intensity (P= 0.05) were also lower in tizanidine group than in placebo. Five patients did not tolerate tizanidine.DISCUSSION: Tizanidine reduced average of headache duration x headache intensity by 50.72 more than placebo in chronic tension-type headache patients. Our findings stress the efficacy of tizanidine in prophylactic treatment of tension-type headache, with ramarkable improvements and few side effects.
