Abstract : Introduction: Inflammation is now recognized to play an important role in all stages of the atherogenic process as a precursor of MI. Hs-CRP is one of the markers of inflammation and statins have both lipid lowering and anti inflammatory effects. It is proposed that aggressive statins treatment can cause lower serum CRP levels. In this study, we compared intensive and moderate atorvastatin treatment effects on mean serum CRP reduction.. Methods: In a double-blind clinical trial 50 more than 21 years old patients who were recently (during last 24 hours) suffered from non-ST elevation MI and and admitted at the emergency or CCU wards of Feiz and/or Chamran hospital (at Isfahan) were entered. They were divided in two equal groups: A. patients under treatment with 80 mg Atorvastatin; and B. patients under treatment with 20 mg Atorvastatin. The measurement of hs-CRP and lipid profile were done at the CCU admission, discharge from the hospital and two months after treatments times. Results: In group A, 12 patients (48 ) were male and 13 (52 ) were female and mean age was 60.1▒13.4 years. In group B, 16 patients (64 ) were male and 9 (36 ) were female and mean age was 59.6▒11.2 years. The mean CRP level at CCU admission (baseline), during discharge and two months latter, was 7▒5.5, 5.3▒4.1 , and 2.3▒4.3 mg/1, respectively in group A, and 7.1▒8.3, 5.7▒7.4, and 3.5▒5.1 mg/I, respectively, in group B. The difference between two groups was not significant during discharge (P=0.32), against two months after treatment (P=0.02). Mean serum LDL level before and after treatment was 134.2▒33.2 and 117.5▒31.9 mg/1 in group A and 141.07▒33.4 and 126.4▒32.5 in group B, respectively. The after treatment difference between two groups was not significant (P=0.77). According to Pearson correlation measurement, there was a correlation between LDL reduction percentage and CRP levels only in all patients totally (P<0.001). not for group A or B patients separately (P>0.05). Conclusion: High dose statins cause lower serum CRP levels due to their anti inflammatory effects, not because of their lipid lowering effects. Key word: Non ST elevation MI, atorvastatin, hs-CRP.
