Introduction:Tissue expansion is a precious adjunct in reconstruction of the defects of the head and neck. There are two methods for Neck tissue expansion, but Common method is subplatysma insertion. This paper compares the results of these two methods considering VSS variables, width of scar and early postoperative complications.Methods and Statistical analysis: During the time period of September 2004 to march 2007, 56 patients who underwent neck tissue expansion in Emam Musa Kazem hospital, included in this descriptive-analytic (experimental) study. We randomized some of these patients in two groups: twenty one patients with subplatysma, and twenty patients with subcutaneous neck expansion. Data gathering was done with the recording of needed variables in a check list that contains VSS variables (scar pigmentation, scar Vascularity, scar pliability, scar height), scar width and early postoperative complications (infection, hematoma, expander exposure, wound dehiscence, flap necrosis and implant failure).Finally, the data will be analyzed by t-test and using SPSS 13. Results: The incidence rate of early postoperative complications in subcutaneous plan is less than subplatysma plan. (P<0.05) Also, the mean of width of scar was 5.45 in subcutaneous and was 7.09 in subplatysma plan. Simple T-Test showed that this difference is statistically significant (t=3.28, P<0.05). But, the mean of VSS was 1.68 in subcutaneous and was 3.48 in subplatysma plan, that Simple T-Test showed that this difference isn't statistically significant (t=2.34, P<0.05)Conclusion: Considering the less postoperative complications rates and width of scar and better aesthetic and functional outcome in subcutaneous plan, it seems that use of neck tissue expansion in subcutaneous plan should continued as a valuable method in reconstruction of post burning injuries in cheek and face.
