Abstract : Introduction: Orchidopexy operation is one of the most common pediatric surgeries. UDT complications are infertility, inguinal hernia, testicular torsion, psychological problems, and inability to routine examination of testes. Common operation for UDT performed after opening of external oblique fascia. The commonest location of testicle in UDT is adjacent to external inguinal canal (superficial inguinal type). In this new method we performed orchidopexy without opening of fascia. In this clinical trial we compared outcomes of classic procedure with new methods. Methods and Patients: This is a clinical controlled trial performed in Al Zahra hospital in 2008-2009. Patients were randomized into either closed technique group (group A, n=52) or open group (group B, n=52). Incidence of short term and long term complications was evaluated by means of a specific questionnaire at the time of surgery and follow up visits. Data were analyzed by SPSS 15.0 software using chi square test and t test. Results: One hundred four patients were included in this study. Prevalence of short term complications (surgical site hematoma, surgical wound infection, and scrotal hematoma), and long term complications (medial thigh sensory loss, loss of Cremasteric reflex, and testicular atrophy) did not significantly differ between two groups. Duration of operation in group A were significantly shorter than group B (p value = 0.000). Conclusion: for surgical management of undescended testes there were no differences between outcomes of open and closed techniques, but operation of UDT without opening the external oblique fascia would perform faster..
