Introduction: Pilonidal disease is a painful condition usually occurring in the intergluteal region. In our hospital (i.e. Al Zahra hospital) the most used procedure for surgical treatment of pilonidal disease is excision and primary closure, but this method could complicated by iatrogenic dead space beneath the surgical site. In this clinical trial we compared two techniques to minimize complications followed by surgery. Methods and Patients: This is a clinical controlled trial performed in Al Zahra hospital in 2007-2008. Patients were randomized into either tie over technique group (group A, n=54) or closed suction drainage group (group B, n=54). Incidence of complications was evaluated by means of a specific questionnaire at the time of surgery and follow up visits. Data were analyzed using SPSS 13.0 software. Results: One hundred eight patients were included in this study. Mean age ▒ standard deviation was 25.08 ▒ 3.6 yr, 94.4 were male, and 5.6 were female. Prevalence of complications (disease recurrence, surgical wound infection, wound dehiscence, and seroma collection) did not significantly differ between two groups. Duration of operation in group A were significantly longer than group B (p value = 0.000).Conclusion: Based on our data in order to surgical management of sacrococcygeal pilonidal disease there were no differences between outcomes of tie over and closed suction drainage techniques..
