Abstract : Background and Aims: From 1937 until 1999, West Nile virus (WNV) garnered scant medical attention as the cause of febrile illness and sporadic encephalitis in parts of Africa, Asia, and Europe. There is no report from Iran about the epidemiology of the WN virus; therefore, we aimed to determine the prevalence of WN virus encephalitis in Isfahan, central Iran. Materials & Methods: This cross-sectional study was conducted from Jan 2008 to Jun 2009 in Alzahra University Hospital, Isfahan, Iran. Patients (n=650) with fever and loss of consciousness coming to the emergency department were consecutively assessed. History of insect bite, organ transplantation, and transfusion, pool contact, and traveling to endemic area in the previous month were assessed. CSF analyses including gram stain, cell count, and biochemistry were done in all patients. Those with evidence of bacterial meningitis were excluded. CSF and serum samples of the patients were centrifuged and stored in -70*c and were then sent to Iran Pasteur Institute (Tehran) for diagnosis of WNV using specific IgM and IgG antibodies and Real Time Poly Chained Reaction (RT-PCR) method. Data were entered in SPSS software version 16.0 and descriptive and analytic statistics were used for analyses. Results: During the study period, a total of 650 patients came to our emergency department with fever and loss of consciousness for whom CSF analyses were done. Bacterial meningitis was diagnosed in 383 patients. Samples of 267 patients were sent for WNV investigation. Serum samples of 12 patients and CSF samples of 6 patients were missed. The remaining patients (n = 249) were 126 (50.6 ) male and 123 (49.4 ) female with the mean age of 35.76 (SD ت‍ 15.37, from 10-81) years. From 249 samples, three (1.17 ) were positive for both serum and CSF RT-PCR, and six (2.35 ) were positive only for IgG antibody. In patients with positive RT-PCR, one (33.3 ) had insect bite in his history (compared with 1.25 of other patients, p = 0.048) and from patients with positive IgG antibody one (16.6 ) had positive history for transplantation and one (16.6 ) for transfusion, compared with 0 and 3.4 of other patients, p = 0.024 and = 0.180, respectively. Conclusion: The results of the present study showed that the prevalence of WNV in our province is 1.17 in patients presenting with fever and loss of consciousness for which bacterial meningitis was ruled out. This is the first report of WNV from Iran and further epidemiological studies are recommended..
