background: Valproic add is a widely used broad-spectrum antiepileptic drug (AED) for treatment of generalized and focal epilepsies. Although several studies have suggested that valproate treatment may be associated with increased body mass index (BMI) and hyperinsulinemia, but some of them had different results. The aim of this study is to investigate insulin resistance and homeostasis model assessment index (HOMA-IR) in Iranian epileptic patients treated by valproic acid in comparison with.patients treated by carbamazepin . Methods:This is a cross sectional study of 30 VPA-treated and 30 CBZ-treated epileptic patients that was done in neurology clinic of Alzahra hospital during 20082009 by convenience sampling, affiliated to Isfahan University of Medical Sciences. Body mass index (BMI) was measured. Biochemical tests include lipid profile, fasting serum insulin, fasting blood sugar (FBS) were done in referential laboratory. HOMA-IR was calculated. The data of two groups were compared by SPSS ver 16 using t student test.Results:BMI was higher in VPA-group than CBZ group (25.7▒ 3.5 vs 21.7▒ 4.1) (P<0.05). Prevalence of obesity were16.6 and 3.3 in VPA-group and CBZgroup respectively. Serum TG in VPA-group (150▒ 77.2mg/dI) was higher than CBZ group (114▒ 35.2 mg/dl) (P=0.023). Serum High density lipoprotein (HDL) level was lower in VPA-group than CBZ-grcup (45.2▒ 11.7 mg/dl vs 54.4▒ 13.9 mg/dl) (P=0.008). Serum insulin, FBS, total cholesterol, and Low density lipoprotein (LDL) had normal values in two groups (P>0.05). HOMAIR(2.05▒0.84,1.88▒0.87 in VPA and CBZ groups respectively) was not statistically significant between two groups (P>0.05).Conclusion: Although VPA therapy is associated with significantly greater BMI and increase in lipid profile, despite several previous studies hyperinsulinemia and insulin resistance didn't occur in any of our cases..
