Abstract

Objective:This study was performed to evaluate and compare graft-take success rate, hearing results and postoperative blunting and lateralization of graft between two groups of patients underwent underlay tympanoplasy; in groupA with elevation of entire annulus and extension of graft onto the anterior bony canal, and in groupB with out elevation of annulus in anterior tympanomeatal angle, and extension of graft forward, against the lateral wall of Eustachian tube. Methods: A randomized clinical study was performed on 63 patients underwent underlay tympanoplasty, 38 cases in groupA and 25 cases in groupB. Sampling method was Random ت Allocation. The study was conducted at a tertiary care referral center (Al'zahra hospital of Isfahan, Iran) from 2007 to 2010. All patients underwent preoperative and postoperative audiogram. Blunting and lateralization of graft were evaluated 6 months after surgery. Data were statistically analyzed using chi-square test, independent t test and ANCOVA (Analysis of CoVariance) test. Significance was set at P < .05.Results: The surgical success rate was 97 in group A and 100 in group B patients without statistically significant difference (P=.79). There were no cases of anterior blunting and graft lateralization in both groups.Averages of postoperative air conduction thresholds in all frequencies were significantly improved in two groups (P < .00.). Closure of ABG was also significant and similar among two groups. The mean ABG was 25.6 dB in group A and 23.4 dB in group B preoperatively that respectively decreased to 8.11 dB and 9.3 dB postoperatively. Conclusion. Elevation of entire fibrous annulus away from sulcus tympanicus and placement of graft against anterior bony canal is not associated with increased risk of anterior tympanomeatal blunting and lateralization of. Success of this technique highly depends to adequate restoration of anterior sharp tympanomeatal angle..
