Some evidence has shown a relationship between human cytomegalovirus(CMV) infection and pregnancy loss. however, whether primary or recurrent CMV infection or altered immune response to CMV is related to recurrent pregnancy loss (RPL) is unclear and few data are available in this ragard. we evaluated CMV infection and humoral immunological response to CMV in women with RPL and compared it to normal pregnant women. this cross- sectional, observational study was conducted in the clinic of clinical immunology, between 2009 and 2010. patients were selected from women referred by outpatient gynecologic clinics to the clinical immunology clinic with complaint of RPL and controls were randomly selected from normal pregnant women referring to obstetric clinic of Alzahra hospital while matching with age. inclusion criteria were age between 18-45 years, at least 3 recurrent spontaneous alortions, and no evidence for organic or othor autoimmune dideases. confirmed by laboratory result as etiology for RPL. blood samples were obtained from patients and controls, centrifuged and stored in -70c and then were transfrred to infectious disease research center of isfahan, central labaoratory. to evaluate CMV IgG and IgG antibodies and IgG avidity index the enzyme linked immunosorbant assay method was used. data were analyzed with SPSS version 16.0, using students t-test, xhi- squared test, and multivariate analyses. there were 43 participants in each group. one case of positive IgM in each group of RPL and controls was detected. also, there were 39 and 30 case of positive IgG in RPL and controls, p< 0.05. patients and controls were similar regarding serumIgG titer. but, IgG titer was significantly higher in RPL group, p<0.05. no differences were found between the two groups in IgG avidity was not related to RPL. also, IgG positivity was related to RPL(P,0.05) and avidity was not related to RPL(p= 0.277). also,IgG positivity (p= 0.159), IgM positivity(p=0.856), or avidity index (p=0.440) were not related to number of avortions in RPL patients. the result of the present study showed that previous exposure to CMV detected by a positive IgG antibody is significantly related to RPL. however, we found no relationship between IgG avidity index and RPL which derives some possilities; 1- recurrent exposure to CMV is a risk fac tor for RPL, 2- other immunological processes like hyper- response to similar number of exposure with CMV or 3- recurrent exposure CMV but inability to detect it by higher avidity index, mostly beacause of altered immune function, are related to RPL. these theories need further investigation..
