Background: The present study examined the effects of Methyldopa on uterine artery diameter, uterine artery blood flow, umbilical artery and fetal middle-cerebral artery in patients with Preeclampsia, using Doppler ultrasound. Methods: The present study was conducted as a double-blind randomized controlled trial on 50 pregnant women, developing mild Preeclampsia. Patients were assessed in two groups: case group received Methyldopa and control group received placebo. Age, BMI, gestational week, the number of pregnancies, pulsatility index, resistance index, systolic/diastolic blood flow ratios of uterine artery, umbilical artery and middle-cerebral artery were evaluated and compared between groups. Results: Studied patients had the mean age of 27.6▒3.3. There wasn't any statistically significant difference between the studied groups in terms of age, gestational age and delivery time variables (p-value>0.05). In case group, uterine artery diameter in patients has significantly increased; while in control group it has decreased. The difference between two groups, however, is not statistically significant, based on the statistical test. Variations in uterine artery diameter of patients in both groups were compared to one another. This value is significantly higher in case group than in control group (p-value<0.0001). Pulsatility and resistance indices and systolic/diastolic blood flow ratio of uterine artery were significantly different between groups (p-values<0.0001) but in fetal middle-cerebral artery no statistically significant difference was observed (p-value>0.05). Conclusions: Based on the findings of this study antihypertensive therapy using a-methyldopa in preeclampsic pregnant women has significant effect on uterine artery in contrast to placebo group..
