Abstract

Introduction: Post-surgical pain control is a high important factor for which several methods have been developed. Recently an interesting and rather accepted theory has been proposed according to which pre-surgical injection of anesthetics and pain relievers can impede the central stimulation leading to pain and as a result reduce the post-surgical pain by inhibiting the production of pain mediators. Regarding to the above mentioned theory, we attempted to figure out if the presurgical local injection of marcaine can affect the pain intensity and duration after hemorrhoidectomy. Materials and Methods: This study is a double-blind clinical trial which was carried out on 2011 at "Alzahra Educational-Medical Center" and "Baharestan" and Milad" hospitals in Isfahan. 50 candidates for grade (III) hemorrhoidectomy were chosen and separated into two 25 member groups by using random diagnostic method. The members of the first group were locally injected with 1.5 mg/kg of 0.5 marcaine before starting the surgery and via intermucus infiltration around the anal site (sample or treatment group). Patients in second group were injected with normal saline in exactly the same conditions and with the same amounts (control group). After the surgery and during post-surgical time intervals of 1, 6,12, 18 and 24 hours, both the pain intensity and the amount of analgesics consumption were recorded in mentioned groups. Obtained data were analyzed using the SPSS software, version 20, t-test and repeated measures ANOVA. Results: At the first 30 minutes after the surgery, the pain intensity in sample group (injected with 0.5 marcaine) was dramatically lower comparing to the control group. At the first and second 6 hours after the surgery, there was also a significant difference in pain intensity between the members of two groups, however, it did not last for the third and fourth 6 hours post-surgical and all the members in both groups felt the same amount of pain during the mentioned time intervals. Analyzing the data using the repeated measures ANOVA test showed that there was a significant difference between the average discrepancies of pain intensity in both groups (P < 0.001). Conclusion: Local injection of marcaine at the anal site in hemorrhoidectomy surgery candidates can reduce the pain intensity, minimally 12 hours after the surgery. Regarding to this and also concerning the fact that the peak amount of pain intensity happens during this time interval, we can suggest patients with no contraindication for marcaine injection to use this drug prior to the surgery..
