Helicobacter pylori is associated with benign and malignant diseases of the upper gastrointestinal tract, and increasing antibiotic resistance has made laternative treatments necessary. our aim was to compare the efficacy of adding N Acetylcysteine (NAC) to standard regimen for Hpylori eradication. we conducted a randomised, open label trial, comparing the efficacy of 14 days of quadruple therapy with amocicillin, bismuth citrate, omeprazole, clarithromycin (group A) versus 14 days of omeprazole, amoxicilline, bismuth citrate and clarithromycineplus NAC (group B) in adult patients with dyspepsia. compliance and side effects were determined by questionnaires completed by the patients. our primary outcome was H pylori eradication, established by negative stool antigen of H pylori one month after the end of treatment. our analysis was by interntion to treat and per protocol. 121 participants aged 21-76 years with mean age of 44.5+/-14.1 land included 52.9 percent female were randomly allocated a treatment. 60 were treated with 14-day standard therapy, 61 with 14-day standard therapy with NAC. the eradication rate in groups A and B with intention to treat analyses was 49/60 and 50/61 respectively. in per protocol analysis, the rate of H pylori eradication in groups A and B was 45/54 and 45/53 respectively. minor side effects were reported in 15 and 21 of A and B groups and only one therapy cessation in group A was created. these side effects well tolerated and consisted mainly of nausea, vomiting and abdominal discomfort. vomiting was significantly more common in B group. standard 14-day triple drug thrapy with NAC is not preferable to standard drug regiments for H pylori infection.
