Pro-inflmmatory Th17 lymphocytes and regulatory T cells (Treg), respectively are the most important players in promoting and controlling multiple sclerosis (MS) disease. plasma exchange therapy (PET) is considered increasingly as an individual treatment in patients of MS relapses, corticosteroid therapy (CST). we compared the frequenc, absolute number and counterbalance of Treg and Th17 cells in eripheral blood of MS patients and healthy controls, then evaluated the effects of PET and CST on these parameters. flow cytometer and heam-analyser instruments were used respectively for subset frequencies and their absolute numbers. 34 relapsed MS patients were compared with 23 healthy individuals, and the effects of PET in 8 patients were compared with the effects of CST in 24 patients under these therapies. our resault showed a decrease in Treg/Th17 ratio in relapsed-MS patients compared to healthy controls and a revese correlation between these subsets in controls byt not in patients. both Treg frequency and Treg/Th17 ratio were negatively corrlated with severity of symptoms. ther was shown to be an enduring increse in Treg frequency associated with MS dissease. PET caused a significant amplification in both Treg freuency and Treg absolute count, without any injluence on Th17 cells. CST enhance the frequency of Tregs without increasing the absolute count of these cells and it caused also a parallel incrase in the frequency of Th17 cells. the Treg/Th17 ratio was significantly enhanced by both PET and CST. CST, but not PET reduced lymphocytes while incresed neutrophils. the nuetropil enhancement was directly correlated with the increase in Th17 frequency. however, only about PE this improvement was sivnificantly correlated with the enhacement in Treg frequency. this study highlights the importance of Treg/Th17 imbalance in Ms attacks. also it suggests thst while CST induces a non-specific decrease in lymphocytes and inhibiting neutrophils from enternig to CNS, PE exerts its improving effect throuth induction of Tregs. This difference confers PET the preferences of more specific and more stable influences on the patient`s adaptive immune system.
