Depression, anxiety and psychological distress affect the emergence, persistence or the severity of functional dyspepsia (FD). The present study was conducted to find the prevalence of FD and its relationships with psychological problems in employees of a large university in central part of Iran. This was a cross-sectional study carried out in Isfahan University of Medical Sciences (IUMS) in 2012. Modified ROME III questionnaire was used to evaluate FD symptoms. Hospital Anxiety and Depression Scale (HADS) was employed to screen for anxiety and depression. 12-item General Health Questionnaire (GHQ-12) was used to assess psychological distress. 4763 employees returned all questionnaires. Logistic regression analysis was used to assess the association of psychological problems and FD in two genders separately. Overall, 654 people (13.7 percent) had anxiety, 1338 ones (28.1 percent) suffered from depression and 1067 people (22.4 percent) had high levels of psychological distress. 710 (15.5 percent) employees were diagnosed with FD based on filled-out questionnaires. It was less common in employees aged <40 vs. in those > 40 years (13.2 percent and 15.6 percent, respectively, P<0.05) and in graduates vs. undergraduates (12.75 percent and 18.4 percent, respectively, P<0.0001). Out of all people with FD, 461 ones (10 percent) had anxiety, depression or psychological distress (OR=3.9, 95 percent CI, 3.3-4.6), and 167 ones had anxiety, depression and psychological distress (OR=4.6, 95 percent CI, 3.75-5.7). Mean scores of anxiety, depression and GHQ in subjects with FD were significantly more than the corresponding scores in those with no FD (P<0.05). The highest scores were recorded in females with FD and the lowest scores in males with no FD. Multivariate logistic regression analysis showed that psychological problems, whether in the form of psychological distress (OR= 2 and 1.3 in males and females, respectively, P=0.0001), anxiety (OR = 2.4 and 2.3 in males and females, respectively) or depression (OR = 2.2 and 1.7 in males and females, respectively, P=0.0001) were significantly linked to FD in both genders. Males consistently demonstrated higher ORs. Subjects with FD were more commonly depressed, anxious or distressed than those with no FD.
