Post extubation coughing is one of the most common complications of general anesthesia which can constitute discomfort and many complications to patient. Laryngospasm, as a complication too, can result in severe outcomes. We compared the effect of small doses of propofol, ketamine and combination of propofol and ketamine on presence and severity of coughing and laryngospasm in patients awakening from general anesthesia. 160 subjects, age 18-75 years, American Society of Anesthesiologists (ASA) class I or II, who were scheduled to undergo operations under general anesthesia were enrolled in the study, 40 in each group. Routine anesthesia was performed. Patient was set at one of the following groups randomly: propofol group (received propofol 0.25 mg/kg), Ketamine (received ketamine 0.25 mg/kg), combination (received propofol 0.25 mg/kg, and ketamine 0.25 mg/kg) and control group (received saline 0.1 ml/kg). During emergence from anesthesia and extubation, coughing and laryngospasm was recorded and graded at previously defined times. The presence of post extubation coughing in combination group was less than other groups and in propofol group was less than control (P<0.05). But the incidence did not differ between propofol group and ketamine, nor did it differ between ketamine and control (P=0.356 and P=0.121 respectively). About severe cough, the difference between combination group and propofol was significant, also between combination group and control (P=0.040 and P=0.006 respectively). There was no significant difference between groups in frequency of laryngospasm (P<0.05). administration of propofol or combination of propofol and ketamine decreases the incidence of post extubation coughing. This combination can decrease severe cases too.
