It has been suggested that proper interaction between reproductive tract and immune system influences on fertility and pregnancy. Recurrent spontaneous abortion (RSA) is a main complication of pregnancy which is usually defined as three or more consecutive pregnancy losses before 20th week of gestation. Different factors are considered as etiology of RSA but in around 50 percent of cases, despite of extensive work up, the cause of RSA remains unknown which called unexplained RSA (URSA). Immunological factors are suggested as etiological factors of unexplained RSA. Innate immunity as the first line of host defense plays an important role in reproduction. One of the most imperative components of innate immunity is pattern recognition receptors (PRRs) which can recognize pathogen associated molecular patterns (PAMPs) derived from various pathogens. Toll like receptors (TLRs) as a main group of PRRs consist of at least 10 functional proteins in human. TLRs are classified according to their location (cell surface and intracellular) and respective ligands. After ligand recognition, TLRs activate intracellular signaling cascades which induce inflammatory and /or anti viral responses. On the other hand, vascular endothelial growth factor (VEGF) is a potent angiogenic factor. It has been shown that VEGF plays important roles in several physiologic processes including reproduction. The aim of the present study was to investigate cell surface TLRs and VEGF gene expressions in endometrium of patients with history of unexplained RSA compared to normal fertile women. In addition, TLR4 protein expression was studied in endometrium of both groups. Also, serum VEGF concentration was accessed and compared between two groups at the same time. Materials and Methods: Endometrial and blood samples were obtained between day 19th and 24th of menstrual cycle (window of implantation) from 10 women with history of unexplained RSA (case group) and 6 fertile women who had at least one successful pregnancy (control group). TLR1,2,4,5,6,10 and VEGF gene expressions were studied by RT-PCR and then quantified by real time PCR. Normalization of these genes expression was obtained by using beta-actin as housekeeping gene. Relative cell surface TLRs and VEGF expression quantities were compared between two groups. Immunohistochemistry (IHC) and enzyme linked immunosorbent assay (ELISA) were used for TLR4 protein expression and serum VEGF assay, respectively. Results: Cell surface TLRs and VEGF gene expressions were detected in endometrial samples of both groups. The mean relative expression of cell surface TLRs genes was higher in URSA group compared to normal women while VEGF gene was expressed lower in endometrium of URSA group. TLR4 protein was detected in the endometrium of both groups .In addition, the serum level of VEGF was significantly higher in URSA group compared to the controls. Conclusion: Alterations in gene expression of cell surface TLRs and VEGF in endometrium and changing of serum VEGF might play important roles in the pathogenesis of unexplained RSA.
