Abstract

|
Pelvic organ prolapse(POP) and related stress urinary incontinence(SUI) are common and have major effects on women's quality of life. We studied the effectiveness and safety of a technique to treat POP and concomitant SUI with a monoprothesis (NAZCA-TC) according to tissue fixation system.Fifty women with SUI and concomitant anterior and or apical vaginal wall prolapse underwent surgery by using a mesh (NAZCA-TC). We anchored the prosthesis to the Sacrospinous ligament for treatment of apical prolapse. Retropubic midurethral sling was performed for treatment of SUI. We used Halfway classification for POP staging, stress test for evaluation of SUI and ICSQ-FLUTS (ICIQ-Female Lower Urinary Tract Symptoms) for assessment of urinary symptoms. Patients were evaluated at 6weeks, 6months, 1, 2 and 3 years after surgery. The mean age of patients was 58.2+/-10.2 years. The mean of follow up was 26.4+/-9.6 months. There was significant reduction in POP staging (P value<0.0001). Success rate of SUI treatment was 87 . There was significant reduction in mean of ICSQ-FLUTS (P value=0.006). There was no significant improvement in urge symptoms (P value=0.29). Mesh erosion was seen in 18 of patients. Sixteen percent of patients have reported significant groin or pelvic pain. Ten percent of patients needed uretherolysis. Monoprothesis (NAZCA-TC) with combined retropubic and transobturator arms and concomitant vaginal SSL fixation has high success rate. It is a safe and effective treatment for POP and simultaneous SUI. It has been shown to be effective for improvement of global lower urinary symptoms.
