| Abstract

Creating access is the most process Percutaneous Nephro Lithotripsy that done by different methods , especially metal telescopic and one shot. in this study two different methods in creating the access namely one shot and telescopic were compared about side effects, amount of bleeding, the amount of radiation received during access creation and the time for creating the access. Material and Methods: in a clinical trial study,240 patients who candidate for PCNL were selected and randomly divided in the two groups. The first group were undergone one shot method and the second group were undergone telescopic method. Amount of bleeding, amount of radiation received during access creation and the time for creating the access was compared between the to groups by SPSS soft ware. Results: The changes of hemoglobin between the before and after intervention in one shot group were 1.08 +- 1.23 and in telescopic group was 1.51 +- 1/08 and no statistically difference between the two groups was seen(P=0.37). The mean of duration of hospitalization in the two groups of one shot and telescopic were relatively 2.36+- 0.67 and 2.28 +- 0.61 days and according to t-test there was no significant difference between the two groups (p=0.37). Average duration of getting radiation in one shot group was 7.8+- 7.7 seconds and in telescopic group was 36.3+-11.5 seconds and there was a significant difference between the two groups (P<0.001). Conclusion: PCNL operation with one shot method is superior due to less time than the telescoping method and for sure this methodology is safer for the patient, surgeon and assistants because of less exposure to radiation, while this approach was not associated with more decrease in hemoglobin level and because of less tissue invasion is superior to telescopic method. Likewise Hospital costs, length of stay and postoperative complications in this method is less therefore it is recommended for patients with indications.
