Acute ankle injury is a common musculoskeletal injury which may be resulted to ankle sprains. This study aimed to compare the efficacy of combination of immobilization and rehabilitation with immobilization without rehabilitation in patients with acute ankle sprain. This controlled clinical trial study was done on 150 patients admitted to Al-zahra and kashani university hospital's emergency department with acute ankle sprain from September 2012 till June 2013. They were divided to two 75 populated groups and were assessed for injury severity recording details such as the ability to weight bear, talar tilt and anterior draw test to assess stability. Radiography was used to exclude fracture .The first group was undergone treatment with splints plus rehabilitation exercises which in second group these exercises were omitted. Patients' physical and mental health statues were assessed incipiently and at the end of the fourth and eighth weeks of the experiment using the SF-36 questionnaire. Results: Physical activity mean scores at 4 and 8 weeks after treatment in the experimental group was significantly better, but physical health limitation in treatment period between the two groups were not significant. Physical pains at onset were similar between the two groups, but during the treatment period, the experimental group had a greater reduction in pain than the control group. General health in experimental group compared to the control group was more favorable (p = 0.048) . Conclusion: To reduce the period of immobilization after ankle sprain to a week then standard rehabilitation exercises to foster the improvement should be undergo.
