Phosphides including aluminum phosphide (AlP) and zinc phosphide (ZnP) are toxic agrochemical pesticides which are commonly used as an agent of self-harm in developing countries. Acute phosphide poisoning is highly lethal poisoning. Because of high toxicity of phosphides and easily availability in our society we evaluated different factors in patients with phosphide poisoning with respect to outcome. Materials and methods: A descriptive analytical study was performed from 2005 to 2014 in the poisoning referral center of Isfahan University of Medical Sciences, Iran. All patients poisoned with AlP or ZnP were investigated. On admission vital signs, clinical manifestation and outcome (survived or death) recorded in a checklist. Results: 60 patients were evaluated with increasing from one patient in 2005 to 15 until 2014. The average age (SE) of patients was 27.61 +- 1.21. 39 patients were men. 96.7 percent of the patients ingested it intentionally. Most of the patients on admission were conscious (66.7 percent). Abnormality of EKG was noted in 8.3 percent. 36.7 percent of the patients died. There was significant correlation between outcome and on admission systolic blood pressure (P=0.001; R= -0.42), O2saturation (P=0.03; R= -0.47), venous sodium bicarbonate (HCO3) (P=0.002; R=-0.42), venous base excess (BE) (P=0.003; R= -0.42), and blood sugar (P=0.03; R= -0.32) Binary backward logistic regression analysis revealed that on admission venous HCO3, base excess, admission systolic blood pressure, and blood glucose were predictive factors for mortality. Conclusion: The rate of phosphide poisoning is increasing during the years. On admission venous HCO3, base excess, systolic blood pressure and blood glucose must be considered as predictive factors for mortality.
