Methadone poisoning is common in clinical toxicology department. Because of easily availability of methadone in our society especially in patients under methadone maintenance treatment (MMT), and long lasting effects of methadone, the outcome of the patients with methadone poisoning was investigated. Materials and Methods:A descriptive analytical study was performed from 2012 to 2013 in the poisoning referral center. All patients with methadone poisoning were investigated. Clinical manifestations and outcome (survived and death) were recorded in a checklist. Factors predict outcome was evaluated by logistic regression analysis. Results: 433 patients with methadone poisoning were investigated. Most of the patients were men (79.52 percent). The average interval between methadone ingestion and the first medical care was 6.57 +- 1.14 hours. 387 patients were survived and 46 patients died. Most of died patients had been under MMT treatment. On admission, the Glasgow coma scale (GCS), systolic blood pressure and respiratory rate was lower in fatal cases. Among all different studied variables, GCS had the prognostic value. Patients with higher GCS on admission had better outcome [OR, 0.47 ; (95 percent CI: 0.38-0.580; P value< 0.0001]. Length of hospital stay was 32.99 +- 25.98 hours. Conclusion: methadone poisoning was more common in patients under MMT treatment. GCS on admission was the most predictive factor in outcome.
