Abstract

Trauma is one of the main causes of morbidity and mortality in children worldwide, leading to about one-third of Emergency Department's (ED) visits among this age group. Since pediatric bone fractures usually damage metaphyseal bone region, which X-ray studies are not efficient to reveal them as fine fracture lines can be concealed with adjacent structures and tissues, there is a necessity for another diagnostic method for this situations. In this study we examined ability of ultrasonography for detecting physeal injuries with normal X-ray findings. Materials and methods: 46 patients of 6-16 year old who were admitted to ED for direct distal radius trauma and had normal X-ray studies were involved in the case group, whereas the same number of patients in the same age group, who were admitted for any reasons other than upper limb trauma, were selected as the control group. For every case, radiographic studies were performed and then reviewed for presence of fractures. Physeal distances in distal radial region of both upper limbs were measured in both study groups using ultrasonography. Mean physeal distance was calculated in both upper limbs in the same points for both groups. Then mean physeal distance's difference of two symmetric regions was calculated and compared between both groups. Results: Mean physeal distance of affected and intact limbs in case and control groups were calculated 0.39 +- 0.02 mm and 0.42 +-0.02 mm respectively, which showed no statistically meaningful difference (p=0.93). Comparing other findings showed no statistically meaningful difference either. Conclusion: Our study suggests that ultrasound studies are not suitable for detecting Salter-Harris fractures of type I and V, thus it's not recommended to treat such patients based on ultrasonographic findings. It's recommended to treat patients with normal radiographic studies who have point tenderness over physeal region, as if they have Salter-Harris injuries; as the physeas may separate or fracture before disruption or spraining of the adjacent strong and flexible ligament.
