Abstract : At the moment, administering non-invasive positive airway pressure (PAP) is comsidered ad the building block for the management of respiratory distress syndrome (RDS). since the beginning of the 1970`s, when nasal continuous positive airway pressure (n-CPAP) established its roots as an interventional approach to treat RDS, there have always been concerns related to the increased work of breathing in newborns treated with this intervention. in this respect, respiratory support systems such as B-BiPAP (Nasal BI-Level positive airway pressure) and SiPAP (sigh-positive airway pressure) has been developed during the last decade. in this study, the two respiratory support systems which, unlike n-CPAP, are categorized ad cycled non invasive ventilation (cycled-NIV) are studied. this study was a randomized clinical trial done on 74 newborns weighing 1500g or less affiliated with respiratory distress syndrome (RDS) hospitalized in NICU at Alzahra hospital from October 2012 to March 2014. patients were randomly assigned to two respiratory support groups of N-BiPAP and SiPAP. each group contained 37 newborns who were compared according to their demographic characteristics, duration of non-invasive respiratory support, the need to aadminister surfactant, apnea incidence, the need for invasive mechanicla ventilation, pneomotrax, intra ventricular hemorrhage (IVH), patent ductus arteriosos (PDA), the duration of the need for oxygen supplement, and chronic lung disease (CLD). the average duration of non-invasive respiratory support, and the average duration of the need for oxygen supplenent had no significant difference between the groups. moreover apnea incidence the need for invasive mechanical ventilation, pneomortax, IVH, PDA< CLD, the need for the second dose of surfactant, and thd death rate showed no significant difference in the two groups. in this study SiPAP shwed no significant clinical preference over N-BiPAP in treatment of the newborns with RDS weighning less than 1500 grams.
