Abstract : tuberculous lymphadenitis (TBL) is the most frequent presentation of extrapulmonary tuberculosis. it is considered a chronic and nagging condition, and tends to respond slowly to efective anti-tuberculous treatmet, this may include enlargement of nodes while on treatment or after cessation of treatment. this is an experimented study in 2008 ti 2013 in the TB disease center of Isfahan province, tuberculous lymphadenitis (TBL) cases unresponsive to systemic conventional treatment been treated by injectable topical antibiotic amikacin as a local infiltration, wheras improvement or regression of glands was evaluated three months after. twenty-eight patients wht TBL were treated by the classic four anti-TB drugs for 6 months (group A), of whom six patients due to lack of response were treated by amikacin as a local injections (group B). there were a significant difference in treatment duration and response between the two groups, that it was better for group B more than A. based on current findings and researchr experiences along the past two decades in the treatment of TBL unresponsive for classic treatment, it seems that local injection of amikacin may be useful in treating such cases. therefore the results of this study is suggested being a pilot research in the treatment of tuberculous lymphadenitis resistant to the conventional therapyand further studies to be taken in this regard.
