Abstract : elevated intracranial pressure (ICP) is a major and potentially lethal disorder in patients admitted in the emergency department (ED). several methods are being used to investigate for elevated ICP. here we assessed and compared the prognostic values of two common tests of ophthalmic examination and ultrasonography in detection of elevated ICP. 131 participants with probable elevation of ICP were enrolled. CT scans, ultrasonography, and ophthalmic examinations were performed. the ICPs in patients with optic nerves thicker than 5 mm and presence of at least one abnormality in ophthalmic examination were considered as elevated ICP. diagnostic accuracy of the two test were assessed by computing the sensitivity, specificity, positive and negative predictive value. the mean+/-Sd age of participants was 46.29+/-10 (77 percent male). the number of diagnosed elevated ICPs with ophthalmic examination and ultrasound were 98 (74.8 percent) and 102 (77.9 percent) cases, respectively. the calculated sensitivity and specificity of ophthalmic examination were 100 (95 percent CI;88.6-100) and 35.4 percnet (95 percnet CI: 26-46.2), respectively. in addition, sensitivity and specificity of ultraound in diagnosis of elevated ICP were 100 percent (95 percent CI: 84-100) and 31.9 percent (95 percent CI: 23-41.7) respectively. the present study revealed that bedside ultrasoun and ophthalmic examination are efficient for screening of patients with probable elevation of ICP. both tests were highly sensitive, but the specificity of them is low.
