Abstract

To evaluate the effect of platform-switched implant placement depth relative to the alveolar bone crest on stress distribution in the peri-implant cortical bone using three-dimensional finite element (FE) analysis. Material and Methods: Computerized models of an Ankylos implant, Ankylos abutment and a block of mandibular bone with 2 mm thick cortical layer were designed. Implant model was placed at 1.0 mm supracrestal to 1.8 mm subcrestal positions at 0.1 mm intervals, comprising 29 FE models. All materials were considered homogenous and isotropic. Conical gold crowns were designed for each model maintaining the occlusal plane at the same level. Models were analyzed under 200N axial and 15░ non-axial loads. Maximum von Mises stress values and distributions were reporte. Results: Cortical bone stress increased only slightly from equicrestal to 0.8 mm subcrestal positions, while supracrestal and deeper subcrestal positions resulted in higher stress values. Subcrestal positions showed maximum stress concentration away from crestal bone. Conclusion: It can be concluded that shallow subcrestal placement of two-stage platform-switched implants increases the stress within the cortical bone only to a limited extent.
