Abstract

Background:In many cases when a patient requires emergency dialysis vascular access is not found and the patient has a temporary hemodialysis catheter is inserted through the jagular vein. However, if they are, these patients require long-term dialysis catheter be temporarily replaced with a permanent catheter cuff. Thus, for these patients with elective surgical replacement of the temporary catheter can be permanent. This means that the previous catheter, a guide wire is passed, earlier catheter removal and permanent catheter cuff is placed. But it is not empty. Apart from the replacement of the catheter under conditions of complete sterility almost impossible and there is always some degree of contamination, placement of the catheter such symptoms of infection are seems to be the former at the increase thrombosis. This study will introduce a new approach that despite a temporary placement, which replaced the previous location and the risk of contamination and infection is at least.Material and Methods:In a clitnical trial study, 50 patients who candidate for catheter placement were selected and randomly allocated in the two groups. In the first group (classic) temporary catheter is placed within a week of the sternocleidomastoid triangle with a permanent catheter guidewire is replaced. In the second group, a temporary catheter inserted through the right mandibular angle and within a week the temporary catheter is removed and the catheter was inserted permanent sternocliodomastoid of the triangle.finally complications of catheterization between the two groups was compared by SPSS soft ware.Results: 10 cases of catheter fitted with a new single attempt was successful, while the classical method, only 6 of the initial effort was successful but no statisticaly difference between the two groups were seen (p=0.21). Complications of catheter fitted with a new technique and classic method was 10 and 3 respectively (12% VS 40%) and there is an statistically difference between the two groups (p=0.024).Conclusion: Based on the findings of this study seem to install temporary and permanent catheters new method more easily and early and late complications is less than the classic method. However, due to the small sample size, it is recommended that more studies are being done in this area
