Abstract

Background: We aimed to examine the value of CRP and fibrinogen levels to predict of these cardiovascular events and compare the predicting power of them between patients with past history of acute coronary syndromes, stroke (ischemic type) and healthy individuals. Methods: In a case control study, 79 patients with past history of acute coronary syndrome, and 88 patients with history of stroke (cerebral ischemia) more than three months ago. Totally, 77 healthy sex and age matched individuals were selected as control group in universities hospital of Isfahan and shahrekord. The patients were selected and followed-up from September 2013 to September 2014 for 3 and 6 months after initial assessment to determine 6-month MACE (major adverse cardiac events). The serum levels of CRP and fibrinogen were measured using ELISA kits. Results: Serum CRP level was significantly higher in the group with acute coronary syndrome than the group with stroke history and healthy group (p =0.045). Using cox regression model (table 2), for history of acute coronary syndrome group increase level of CRP (HR=1.29 (1.01-1.66) (P value=0.038) and fibrinogen (HR=1.01 (1.01-1.02) (P<0.001) and in the stroke group level of CRP (HR= 1.61 (0.97-2.67) (P=0.065) and fibrinogen (HR=1.02 (1.01-1.04))(P value=0.010)and normal group group level of CRP (HR= 2.06 (0.71-5.99)) (P=0.183) and fibrinogen (HR=1.01 (0.99-1.04)) (P=0.294) after adjust for age and sex have not observed any changes in results of study, so the group of history acute coronary syndrome and stroke history could effectively predict 6-month MACE in crude as well as age and sex adjusted model. Conclusion: Our study achieved two important findings, first of all showed the higher values of these biomarkers able to predict MACE, even after inclusion of baseline covariates. A high value of the increased level of CRP and fibrinogen predicts recurrent cardiovascular events in patients with past history of cerebrovascular ischemia and acute coronary syndrome measured after evaluating the acute phase and their related outcome. In addition higher levels of both CRP and fibrinogen markers in those patients who have past history of acute coronary syndrome and stroke compared with healthy individuals
