Abstract : Background:Few studies have so far been conducted concerning the effect of remifentanil infusion on emergence agitation in children undergoing Adenotonsillectomy. Therefore, the present study aimed to determine the effect of adding remifentanilinfusion to Isoflurane and reducing the concentration of Isoflurane on the reduction of agitation recovery in children undergoing Adenotonsillectomy.Material and Methods: In a clinical trial study, 86 patients who candidate for Adenotonsillectomy were selected and randomly divided in two groups. The remifentanil groupwas maintained with 0.4%isoflurane andcontinuous infusion of remifentaniland the isoflurane groupwas maintained with 1.2% isoflurane for anesthesia and incidence emergence agitation was determined and compared between the two groups.Results: During the study, 29 patients suffered from agitation that out of them, 10 cases were in the group receiving Remifentanil and 19 cases in the group receiving isoflurane (23.3% vs. 44.2%) and the incidence of agitation in the group receiving isoflurane was significantly more (P<0.001). Conclusion:Remifentanil infusion in children undergoing Adenotonsillectomy reduces the consumption of isoflurane and the incidence of agitation and delirium after the surgery and that its use in children undergoing Adenotonsillectomy is recommended providing the view of the surgeon and anaesthesiologist and no contra indications for use
