Abstract

Background: ASV was introduced in 1994as an"electronic ventilator protocol" that incorporate s measurements of respiratory mechanics and algorithms of closed-loop pressure control to maintain a target minute ventilation. At present, the conventional method for ventilation is Supported by accommodative or ASV that the latter method is done with two settingASVmn110% and ASVmv120%. ASV is a new method for its ease of use and speed at the time of extubation is More considerable. The aim of this study was to compare differences in duration of mechanical ventilation and hemodynamic changes during recovery and length of stay in intensive care units with these two setting.Material and methods : This is a double-blind clinical trial study which is done in Ayyatollah Kashani medical center in Isfahan. 40 patients candidate for ventilation were selected and included in this study. Patients on admission to ICU were randomly divided into two groups of A and B. All patients were ventilated by Rafael ventilator. Ventilator parameters were set on ASVmv110% or ASVmv120% by another person, and patients' monitoring which included pulse oximetry, ECG monitoring, CVP and arterial pressure, was done on both groups. Finaly the data entered to computer and analyzed by SPSS soft ware. Results: The time average of connecting to ventilator in two groups in modes of 110% and 120% was 12.3±3.66 and 10.8±2.07 days respectively, and according to t-test, there was no significant difference between two groups (p=0.11).The average of length of stay in ICU in two groups of 110% and 120% was 16.35± 3.51 and 15.5±2.62 days respectively, and according to t-test, there found to be no significant difference between two groups (p=0.41).Conclusion: Using of ASV120% can decrease of separation time compared with ASVmv 110%. Also there isn’t a considerable side effect on hemodynamic of patients. Due to the limitations of this study suggested more extensive studies with larger sample size are needed.
