Background:Cardiovascular disease is the first cause of death in the world and Iran. Aims of this study are determent predictive factors of 28-day survival rate from acute myocardial infarction in earlyand latepatients in Isfahan and Najaf Abad County, Iran. Methods: In a cohort study based on hospital records during 1999-2008, all patients (n=14426) with an AMI admitted to all hospitals in Isfahan and Najaf Abad were investigated. To determine predictive factors of short-term (28-days) survival in early and late patients, we calculate unadjusted and adjusted hazard ratio withuni-variate and multi-variate Cox regression. Results: Short-term (28-day) survival rate in Early patients were 96.64% and in Late patients 89.42% (p<0.001).InEarlypatients80% andin late patients79.3% of mortality occurred in the first 7 day of disease occurrence. Hazardratio ofdeathin two group in women was higher,so in Earlypatients was (HR=1.97 CI 95% 1.32-2.92)and in Late patients (HR=1.35 CI 95% 1.19-1.53) compared to men.Hazardsratio ofdeathhasrisen trendin two groupwith increasing age. Conclusion: Short –term survival rate in Early patients are higher than Late. In addition, case fatality rate from AMI in women are higher than men, in both group (Early and Late patients): sex, age, an atomic location of myocardial infarction based (ICD10), Cardiac enzymes, and clinical symptoms are significant predictors of survivalin Early and Late patients following acute myocardial infarction.
