Abstract
|
:
|
Purpose. To compare pulsed and continuous accelerated corneal cross-linking for keratoconus. Methods. 70 eye of 70 patients with progressive keratoconus were included in this prospective interventional study. The patients randomly divided into two groups, each individual underwent a single method for both eyes, if needed. But, for the patients whose both eyes underwent the procedure, the information of one eye was randomly selected for the study. All patients underwent cross-linking after nearly total epithelium removal and impregnation of cornea with riboflavin 0.1% dextran-free solution with a KXL UV-A source (Avedro Inc., Waltham, MS, USA). The exposure was set at 30 mW/cm2, corresponding to a total energy dose of 7.2 J/cm2. 36 eyes were enrolled in the pulsed group, and received irradiation using pulsed light (1 second on, 1 second off) for 8 minutes, whereas 34 eyes underwent continuous irradiation for 4 minutes. Patients were evaluated before and at 3 month after the procedure. Results. Pre-operative uncorrected Log MAR visual acuity was 0.83 +/- 0.48 for the pulsed, and 0.88 +/- 0.45 for the continuous irradiation groups, 0.71 +/- 0.36 and 0.84 +/- 0.43 at 3 months after the procedure, respectively. Anterior segment optical coherence tomography demonstrated a significantly deeper stromal demarcation line in the pulsed irradiation group when compared to the continuous group (201.11 +/- 27.76 μm, versus 159.88 +/- 20.86 μm, p<0.001).Conclusion. This study showed that pulsed accelerated corneal cross-linking induced a significantly deeper demarcation line in the cornea, when compared to continuous accelerated cross-linking at 30 mW/cm2
